what is the recommended chimney opening height from the floor in the kitchen?

where should be it placed - exactly on top of the stove or to the right of it? If right, what should be the offset?

As per my knowledge the chimney opening is kept above the planned stove position. Just make sure that you are not making windows behind stove as the chimney sits on the wall behind stove. I had seen in one house where the window needed to be blocked for fixing the chimney.
