Read an article in Deccan that the forms are available in all nationalised banks. But I am not able to find them. I have already tried Canara and sbi. Can someone tell me where to get one?

there is a mis-match between the plan mentioned on the board and the actual sites. in the plan the site numbers are in one order while the actual site has got the numbers in reverse order. is it a matter of concern????
